1. Must own & operate a valid retail business
2. Must be a brick and mortar storefront (not just a PO Box)
3. Must submit a copy of your resale exemption certificate (first orders only)
4. Must sign/execute the Terms and Conditions. Return via email a copy with original Signatures to  Wholesale@madikaydesigns.com.

Note: By submitting an application to become a Madi Kay Wholesaler you certify that you are the owner or an authorized representative for the above business and that you qualify on all counts to be a Madi Kay Designs wholesaler, and that the transaction being made is a wholesale, tax exempt purchase for resale.
